The Effects of Propranolol on Preoperative Preparation in Thyrotoxicosis

Abstract
42 patients with thyrotoxicosis have been preoperatively prepared with beta-adrenergic receptor blocking agent, propranolol(lnderal), at the Department of Surgery of Chonnam National University Hospital.
The results were as follows.
1) Male to female ratio was 1:9.5 and majority-(71.4%) were in 3 rd and 4 th decade.
2) Duration of symtoms and signs of 35 patients(83.3%) was over 6 months.
3) Main symptoms and signs were in the order of decreasing frequency; neck mass(100%),. tacbycardia(95.2%), nervousness (90. 4%), sweating(83.3%).
4) Pulse rate decreased from 107 per mm. preoperatively to 77 per mm. postoperatively.
5) Thyroid function test revealed that serum T©ý and T©þ were high level above normal in 41(97.6%) patients and 24 hours I^(131) uptake showed all above the normal value. On thyroid scan, diffuse enlargement of thyroid gland(40 patients) and nodular enlargement in 2 patients? were noted.
6) Electrocardiographic findings were sinus tachycardia(42.9%) to normal(26.2%), sinus? tachcardia and left ventricular hypertrophy(9.5%), atrial fibrillation(2. 4%).
7) In preoperative preparation, propranolol only were most frequently used(32 patients),. and others were propranolol plus methimazole, propranolol plus methimazole plus Lugol¢¥s Solution and propsanolol plus Lugol¢¥s solution.
8) The type of operation was bilateral subtotal thyroidectomy. The weight of resected spe-cimen was 50~60 gm and that of remnant was 5 gin.
9) In histologic classification, 35(83.3%) cases were diffse toxic goiter, 3(7.1%) cases were nodular toxic goiter, each 2(2.4%) cases were toxic adenoma and thyroiditis.
10) The postoperative complication as follows, temporary hypocalcemia 4 cases, hypothyroidism 3 cases. There was no thyroid crisis, recurrent laryngeal nerve injury and death.
